What is this document?

A laptop transfer form is an official internal company document that records the fact of issuing a corporate laptop or computer to a specific employee. The document identifies the device (make, model, serial number), describes its condition at the time of issue, and records the employee's responsibility for keeping the equipment safe.

The laptop transfer form protects the employer: without it, it is impossible to prove who has the device and what condition it was issued in. It also protects the employee: if the device already had scratches or defects at the time of issue, this is on record.

The document is used not only for laptops but for any corporate equipment with a serial number: tablets, smartphones, monitors, cameras.

When do you need it?

A laptop transfer form is needed:

When onboarding a new employee. Equipment is issued on the first working day — the form is signed at the same time.

When replacing or upgrading equipment. The old device is returned with a form, the new one is issued with a form.

When an employee transitions to remote work. It is especially important to record exactly what the employee has taken home.

When equipment is returned upon dismissal. Compared against the original issue form.

When equipment is transferred between employees or departments. A document of who transferred what to whom and in what condition is always required.

Common mistakes

Most common mistakes when completing a laptop transfer form:

Issuing equipment without any documentation. Without a form, the company has no records of who has which device. Upon dismissal, the employee can claim they received nothing.

Not listing accessories in the form. A missing charger or bag at return becomes the subject of a dispute. List every accessory separately.

Skipping the serial number. Without a serial number it is impossible to prove that a specific laptop is the same one that was issued. Make and model alone are not sufficient identification.

Not recording the device's condition at issue. If the laptop had a cracked casing or faulty keyboard — this must be in the form. Otherwise a conflict arises at return.

Not completing a return form. Upon dismissal or equipment replacement, a separate return form is needed — or a signature on the original issue form.

What must be included?

  • Employee's full name, job title, and department
  • Company name and full name of the responsible representative (HR or IT manager)
  • Device make and model (e.g. Apple MacBook Pro 14", 2023)
  • Device serial number — mandatory
  • IMEI or company inventory number (if applicable)
  • Condition at time of transfer: new / excellent / good / satisfactory / damaged (description)
  • Full list of accessories: charger, cable, bag/case, mouse, adapters
  • Installed software or configuration (if required)
  • Date of transfer
  • Employee obligations: return in working order, not use for personal purposes (if such a policy exists), report faults
  • Liability for damage or loss
  • Signatures of both parties

Is a digital laptop transfer form legally valid?

Yes. Under eIDAS Art. 26 a digitally signed document is equivalent to a paper one in all EU countries. Pakto signs using ECDSA P-256 from the Secure Enclave, with a TSA timestamp and verified identity — sufficient for employment disputes, insurance claims, and audits.

What happens to the form when an employee is dismissed?

The original issue form documents what was transferred and in what condition. At return, a return form is completed or the employee signs a confirmation of return. The digital record in Pakto allows instant comparison of what was issued and what was returned — even years later.

Can this template be used for other equipment?

Is it mandatory to include the device serial number?

Yes, the serial number is mandatory. It is the only way to prove that the specific laptop in the employee's hands is the same one stated in the form. Identical makes and models are not identification.

What if the laptop was damaged or broke during use?

The transfer form records the condition at the time of issue. If damage occurred during use, the description in the issue form is compared with the condition at return. Having a signed document simplifies the insurance claim and resolution of employment disputes.
